Answer the following questions honestly to gain insights into potential ADHD symptoms. Note that this is not a diagnostic tool, and consulting with a healthcare professional is recommended for an accurate diagnosis.

  • Are you easily distracted by unrelated stimuli or prone to forgetfulness in daily tasks? YES OR NO

Hyperactivity:

  • Do you often feel restless or find it difficult to stay seated in situations where it's expected? YES OR NO

  • Are you constantly moving, tapping your hands or feet, even in situations where it might be inappropriate? YES OR NO

Impulsivity:

  • Do you tend to act on impulses without considering potential consequences? YES OR NO

  • Is interrupting others during conversations a common behavior for you? YES OR NO

Organization and Time Management:

  • Do you struggle with organizing tasks and activities, often procrastinating until the last minute? YES OR NO

  • Is punctuality a challenge for you, frequently running late to appointments or meetings? YES OR NO

Task Completion:

  • Do you often start tasks but struggle to finish them before moving on to something else? YES OR NO

  • Are you easily bored with routine or repetitive work? YES OR NO

Forgetfulness:

  • Do you frequently misplace items necessary for daily activities? YES OR NO

  • Are you forgetful in daily activities, even when they are routine? YES OR NO

Relationships and Social Interactions:

  • Do you find it challenging to listen attentively during conversations? YES OR NO

  • Are there difficulties in maintaining focus during social interactions? YES OR NO

Work and Academic Performance:

  • Have you experienced challenges in meeting deadlines and completing assignments at work or school? YES OR NO

  • Do you often switch between tasks before completing them? YES OR NO


Scoring:

  • For each "Yes," give yourself 1 point.

  • Add up your points to assess your ADHD symptom severity.

Interpretation:

  • 0-4 points: Low likelihood of ADHD.

  • 5-8 points: Moderate likelihood; consider consulting with a healthcare professional.

  • 9 or more points: Higher likelihood; seeking professional evaluation is recommended.


Remember, this self-assessment is not a substitute for a clinical diagnosis. Consultation with a healthcare professional is essential for a comprehensive evaluation and guidance on appropriate interventions.
